A stroke does not usually develop gradually over several days. Most often, symptoms appear suddenly, within minutes—and sometimes seconds. It is this sudden change that distinguishes a stroke from many other medical conditions.

The most common signs are drooping corner of the mouth, weakness or paresis on one side of the body, speech disorders, as well as sudden problems with vision, balance, and coordination. They do not all have to occur at the same time. In practice, already one sudden neurological symptom All you need to do is treat the situation as a suspected stroke and call an ambulance.

A typical scenario looks like this: the patient suddenly begins to slur their speech, is unable to lift one arm, has a „crooked” face, or wobbles when trying to stand up. Sometimes they also have trouble understanding a simple question, even though the conversation had been completely normal just moments before. These are classic responses to the question, How to Recognize a Stroke at home or at work.

The UDAR/FAST Test, Step by Step

The simplest approach is to use a short screening test known as FAST or, in Polish, UDAR. It does not replace a doctor or a CT scan, but it allows you to identify symptoms that require an immediate response in 30–60 seconds.

U – smile: Ask the patient to smile. If one corner of the mouth droops or the face is noticeably asymmetrical, that's a red flag.

D – palms or hands: Ask the person to raise both arms in front of them. If one arm drops, does not rise at all, or is noticeably weaker, this may indicate paresis.

A – articulation: Ask the person to repeat a simple sentence, such as, „It’s a warm day today.” Slurred speech, stuttering, mixing up words, or a failure to understand the instruction are very concerning.

R – Respond immediately: If any of the symptoms appear, call 112 or 999. Do not try to determine on your own whether „this is definitely a stroke.”.

It's worth adding two more elements to this test that often occur with a stroke: sudden loss of vision and balance disorders. The patient may experience double vision, have a blurred field of vision, or suddenly be unable to take a few steps without staggering. This is particularly important because not every stroke causes a noticeable drooping of the corner of the mouth.

In practice, the question „How to Recognize a Stroke” should be replaced with a simpler rule: if there’s a sudden change in facial appearance, speech, strength in the arms or legs, vision, or balance, react immediately. Don’t wait for a second symptom, and don’t assume that „it will go away in a moment.”.

Less common but still urgent symptoms

Not every stroke fits the textbook description. A hemorrhagic stroke, in particular, may begin a sudden, very severe headache, described as the strongest of his life. They may join nausea, vomiting, dizziness, confusion, drowsiness or even loss of consciousness.

Some patients experience sudden disorientation: they don’t know where they are, don’t understand simple instructions, or behave differently than usual. These symptoms are sometimes mistaken for fatigue, low blood sugar, an infection, or a „momentary fainting spell.” This is a mistake, because a stroke can indeed present with precisely these nonspecific symptoms.

Exercise particular caution with younger people. Although stroke is mainly associated with older adults, about 30% patients are people under 65 years of age. Therefore, sudden neurological symptoms must not be attributed solely to stress, migraines, lack of sleep, or tension.

⚠️ Just one symptom is enough: Don't wait for all the symptoms to appear. A drooping corner of the mouth or sudden slurred speech is reason enough to call 112 or 999.

What to Do Immediately If You Suspect a Stroke

When a stroke is suspected, speed and a simple plan of action are paramount. Call 112 or 999 right away. Do not drive a sick person „at your own pace” in your own car if the symptoms are obvious. The emergency medical team can begin the assessment while en route and transport the patient directly to the appropriate hospital.

The worst decision is to wait and see for 20–30 minutes „just in case.” In the case of a stroke, that time can determine whether causal treatment will be possible. If you’re wondering, How to Recognize a Stroke And what should you do next? The answer is: recognize the warning signs and call for help immediately.

The first 5 minutes after symptoms appear

In the first few minutes, do only what is absolutely necessary. Sit or position the patient in a safe position, preferably with the upper body slightly elevated, if they are conscious. If the patient is vomiting or has impaired consciousness, ensure their airway is clear and position them safely on their side, if the situation requires it.

Call an ambulance,

Check what symptoms occurred and at what time,

Have your ID and a list of your medications ready, if you have them on hand,

Do not leave the sick person alone.

If the patient is conscious, speak briefly and calmly. Don’t ask too many questions at once. A person who has had a stroke may not understand you or be able to answer, even if they are conscious. This isn’t stubbornness; it’s a possible symptom of damage to the speech centers.

What time do you need to remember?

The key is time of onset of symptoms. If you witnessed the moment when speech difficulties or paralysis first appeared, note the exact time, such as 8:40. If you don't know exactly when it started, indicate the moment when the patient was recently asymptomatic. It could be 10:00 p.m. if he was found with paralysis that morning.

This information affects eligibility for treatment, particularly thrombolysis and thrombectomy. From the doctor’s perspective, what matters is not the time the ambulance was called, but rather the onset of the problem—determined as accurately as possible—or the last time the patient’s condition was normal.

What Not to Do Before the Ambulance Arrives

Do not give food or drink, because the patient may have difficulty swallowing and could choke.

Do not administer medication on your own, including aspirin, unless specifically prescribed by a doctor. Without imaging tests, you won't know whether it's an ischemic stroke or a hemorrhagic stroke.

Don't make the sick person walk or „get moving” if they have weakness or dizziness.

Don't wait for things to get better, even if the symptoms subside somewhat after a few minutes.

Don't conduct a long-term home surveillance operation And don't try to diagnose the problem over the phone with your friends.

This last point is also important in the case of a TIA, or transient ischemic attack. Even if the symptoms disappeared after 10 or 30 minutes, the situation still requires urgent diagnostic evaluation at a hospital.

✅ Note the time when symptoms began: If you don't know the exact time, please indicate when the patient was last symptom-free. This is important for determining eligibility for treatment.

Why Every Minute Counts

When it comes to a stroke, “time is of the essence” isn’t just a slogan—it’s a hard medical fact. The longer a part of the brain is deprived of a normal blood supply, the greater the risk of irreversible damage. That’s why the question is, How to Recognize a Stroke, is always linked to the other: how quickly you can get help.

The consequences of the delay are very real. The stroke remains in Poland the leading cause of permanent disability among adults. The data also indicate that in 2023, there were approximately 4,000 deaths in a week from an ischemic stroke, approximately 9,000 per month and approximately 14,000 in the long term. This illustrates the scale of the problem and the cost of delay.

Time window for thrombolysis

In ischemic stroke, the standard causal treatment may be intravenous thrombolysis, that is, the administration of a thrombolytic agent. Current guidelines state that this should generally be initiated up to 4.5 hours from the onset of symptoms, provided the patient meets the safety criteria.

In practice, this doesn’t mean you have 4.5 hours to think things over at your leisure. First, you have to call an ambulance, get to the hospital, undergo a CT or MRI scan, have lab tests done, and receive a neurological evaluation. So the realistic window for delaying treatment at home is very small.

The most commonly used method for thrombolysis is Alteplaza, and, increasingly, also tenekteplase. For the patient, one thing is most important: the sooner you get to the center, the greater the chance that the doctor will even be able to consider this type of treatment.

When Is a Thrombectomy Indicated?

In some patients, it is possible to mechanical thrombectomy, which is an endovascular procedure involving the removal of a blood clot from a large cerebral vessel. The standard time window is usually about 6 hours in eligible patients, although the decision depends on the type of occluded vessel, the patient's condition, and the results of imaging studies.

This treatment isn't suitable for everyone, but for the right patients, it can be a game-changer. The problem is that even the best technology won't help if the patient arrives too late or if the symptoms were ignored during the first few hours.

Stroke during sleep and unknown time of onset

The situation becomes more challenging when a stroke occurs during sleep or the exact time of onset is unknown. This does not automatically mean that treatment is not possible. In such cases, doctors can use advanced perfusion imaging, which helps determine whether some brain tissue can still be saved.

This is important information because many family members mistakenly believe that since the patient woke up with symptoms, „it’s already too late anyway.” It isn’t. Immediate action is still necessary, because modern diagnostic methods make it possible to qualify certain patients for treatment even outside the standard protocol.

Ischemic Stroke vs. Hemorrhagic Stroke and the Scale of the Problem in Poland

A stroke is not a single disease with an identical mechanism. The most important classification includes ischemic stroke i hemorrhagic stroke. For a witness to the incident, both require the same response: an urgent call for help. However, they differ in their cause, progression, and method of hospital treatment.

Ischemic stroke

Ischemic stroke accounts for approximately 80–90% of all strokes. It occurs when an artery supplying blood to the brain becomes blocked, most often by a blood clot. A portion of the brain stops receiving oxygen and glucose, and nerve cells quickly begin to die.

Symptoms depend on which part of the brain is affected. That is why one person may experience primarily speech disorders, another may have leg paresis, and yet another may have sudden vision problems. This is precisely what causes How to Recognize a Stroke It must be understood broadly, not just through the lens of a single typical symptom.

Hemorrhagic stroke

A hemorrhagic stroke accounts for approximately 10–20% cases. It occurs as a result of a blood vessel rupture and bleeding into the brain or surrounding areas. This type of stroke often has a more sudden onset and is more likely to result in a sudden, severe headache, vomiting and disturbances of consciousness.

In a home setting, you probably won’t be able to tell the difference between stroke types without imaging tests. And that’s exactly why you must not give antiplatelet or anticoagulant medications „just in case.” A hospital evaluation is necessary first.

There are also very rare venous strokes affecting the cerebral venous sinuses. They account for less than 1% in all cases. This is a statistical outlier, but for the patient, it is still an urgent condition requiring a specialist evaluation.

What is the scale of the problem in Poland?

The scale of the phenomenon is significant. In Poland in 2023, there were 72,252 cases of ischemic stroke. Of that number, as many as 89% were strokes first-time. This means that for most patients, the crisis unfolds without any prior „training” and without the family having any experience in recognizing the symptoms.

It’s also worth remembering that this problem doesn’t affect only the elderly. A significant proportion of patients are people who are still working. That’s why a stroke is not only a medical emergency but also a common cause of long-term dependence on others, rehabilitation, and a loss of independence.

Mini-stroke and Risk Factors You Must Not Ignore

Not every neurological episode results in a full-blown stroke, but that doesn't mean it should be taken lightly. It is especially important to understand what TIA, or transient ischemic attack, commonly known as a "mini-stroke.".

Can a mini-stroke go away without a trace?

TIA symptoms usually resolve within 24 hours, often much more quickly. This may include weakness in the arm lasting a dozen or so minutes, temporary slurred speech, brief vision problems, or sudden facial asymmetry that disappears after a while. However, this is not a „false alarm.”.

A TIA is considered a serious warning sign. It is estimated that approximately 1 in 5 people After such an incident, the patient may suffer a more severe stroke in the coming days or weeks. In other words: the symptoms have subsided, but the risk remains—and in some cases has even increased.

So if someone asks, How to Recognize a Stroke, there is one caveat: even transient stroke-like symptoms require urgent diagnosis. This is not a condition you can wait out until Monday or until a scheduled appointment a month from now.

The most important risk factors that can be mitigated

There are some risks you can't control, such as age or certain family circumstances. But there are many factors you can realistically reduce. The most important ones are:

hypertension – one of the most significant factors causing damage to blood vessels,

diabetes – accelerates vascular changes,

high cholesterol – contributes to atherosclerosis and narrowing of the blood vessels,

smoking cigarettes – increases the risk of blood clots and damages the vascular endothelium,

obesity,

lack of regular exercise,

unhealthy diet with too much salt, trans fats, and highly processed foods.

In practice, prevention isn’t about a single „miracle” solution, but rather the sum of simple decisions: monitoring blood pressure, maintaining blood sugar levels, quitting smoking, managing cholesterol, losing weight, and exercising regularly. Even a 30-minute walk 5 days a week is a concrete step, not just a vague idea.

What is hospital treatment like, and what is known about hydrogen inhalation?

Upon arrival at the hospital, the most important thing is to quickly determine what type of stroke you are experiencing. It is impossible to determine this reliably based solely on symptoms. That is why the standard procedure is to perform an urgent diagnostic imaging, most commonly computed tomography, and in certain situations also magnetic resonance imaging and more advanced vascular or perfusion imaging.

Standard of care according to current guidelines

If an ischemic stroke is diagnosed and the patient meets the criteria, doctors may implement intravenous thrombolysis. The current guidelines list the following as the primary medications: Alteplaza and, increasingly, tenekteplase. This treatment is intended to dissolve the blood clot and restore blood flow.

In patients with occlusion of a major vessel, the following is considered mechanical thrombectomy. Simply put, this is a procedure performed through the blood vessels, without the traditional opening of the skull. The doctor inserts a catheter and removes a blood clot from the artery. In appropriately selected patients, this can significantly improve the functional prognosis.

In addition to treating the underlying cause, the hospital also focuses on stabilizing respiration and circulation, monitoring blood pressure, blood glucose levels, and body temperature, assessing swallowing function, preventing complications, and initiating rehabilitation as soon as possible. This is important because stroke treatment does not end with a single procedure.

Hydrogen Inhalation: What the Research Says

The topic of hydrogen inhalation is coming up more and more often as a potential adjunct to therapy. In acute ischemic stroke, a study was published randomized clinical trial, in which inhalation therapy was administered between between the 6th and 24th hour since the onset of symptoms. The results indicated security this method and improvements in some imaging and clinical indicators, including MRI and the scale NIHSS and Barthel Index.

These are interesting findings, but they must be interpreted with caution. We have a limited number of studies at our disposal, and there is a lack of large, multicenter analyses confirming the effect of hydrogen inhalation on experience, long-term efficacy, and treatment outcomes in a broad patient population. Therefore, it is not currently a standard of care on par with thrombolysis or thrombectomy.

The most straightforward conclusion is simple: hydrogen inhalation can be considered today as potential support under study, but not as a substitute for prompt diagnosis and hospital treatment. If you’re interested in this field, you first need to understand the basic sequence of actions: recognizing symptoms, calling for help, transporting the patient to a stroke center, and providing treatment in accordance with guidelines.

⚠️ Do not delay standard treatment: Hydrogen inhalation is being studied as a supportive treatment. It is not a substitute for calling 112/999, thrombolysis, or thrombectomy.

Frequently Asked Questions

The most important takeaway is simple: if you experience sudden weakness, facial asymmetry, or problems with speech, vision, or balance, don’t wait a single minute. Quickly recognizing the symptoms and calling for help immediately offers the best chance for effective treatment and minimizing the long-term effects of a stroke.

Learn more - Click here: https://anev.com.pl/

Czego się dowiesz?
