In practice, the diagnosis covers two main areas: difficulties in social communication and interactions and repetitive, limited patterns of behavior, interests, or activities.

The word „spectrum” is key here. It does not refer to a single, rigid set of symptoms, but significant variation. One person may speak very fluently, hold down a job, and mainly need support with relationships or sensory overload. Another may have significant difficulties with speech, independence, and daily functioning. Both fall under the same diagnosis of ASD, or autism spectrum disorder.

Why Is ASD Being Discussed Today?

In current medical classifications, that is, DSM-5, DSM-5-TR i ICD-11, the general term “ASD” is used. It stands for “autism spectrum disorder.” Previously, separate terms were used, such as childhood autism or Asperger’s syndrome. Today, they are not classified as separate diagnostic entities, because research and clinical practice have shown that a single broad diagnosis—which assesses the severity of difficulties and support needs—better reflects reality.

This is also an important change for parents and adults seeking answers. Instead of focusing on the label, experts today assess specific operational profile: communication, social interactions, behavioral flexibility, sensory responses, language, independence, and co-occurring difficulties.

What Does "Diverse Support Needs" Mean?

If you're wondering, What is the autism spectrum? In practice, the simplest way to put it is this: two people with ASD may share the same diagnosis but have completely different everyday needs. One person may need simple accommodations at school or work, such as a quieter space, clear rules, and a predictable schedule. The other may need intensive speech therapy, support with self-care, and assistance from a caregiver.

„Spectrum” also includes strengths. Some individuals demonstrate very strong skills in analytical thinking, remembering details, working according to rules, technology, music, or art. Therefore, a thorough assessment does not end with a list of difficulties. It should also highlight the strengths that can be leveraged in education, therapy, and professional life.

💡 ASD is now considered a single diagnosis: ICD-11 and DSM-5 no longer distinguish between Asperger's and childhood autism. Both fall under a single diagnosis: ASD.

What are the current diagnostic criteria for ASD?

The current diagnostic criteria provide a very specific framework for this topic. According to DSM-5 and DSM-5-TR A diagnosis of ASD requires the presence of symptoms in three areas of public communication and at least two of the four types of repetitive and restricted behaviors. In addition, the symptoms must have been present since early childhood—even if they do not become apparent until later—and must genuinely interfere with functioning at home, at school, at work, or in relationships.

ICD-11 is fundamentally consistent with this, but it organizes the description of functioning even more clearly. In practice, factors such as the following are taken into account:, Is the child's speech age-appropriate?, whether there is intellectual disability and what they look like sensory responses. This makes the diagnosis more useful, because it not only identifies the difficulty but also helps in selecting the appropriate support.

Three Areas of Public Communication

The first pillar of the diagnosis covers social communication and interactions. It involves three mandatory areas that must all be present:

Difficulties with Social-Emotional Reciprocity – for example, limited ability to keep a conversation going, less willingness to share interests, and difficulty engaging in a natural „give-and-take” during interactions.

Difficulties in Nonverbal Communication – This may involve eye contact, gestures, facial expressions, tone of voice, or understanding others' cues.

Difficulty in building, understanding, and adapting relationships – for example, difficulties with adapting behavior to a situation, understanding social rules, „pretend play” in children, or functioning within a group.

It doesn’t have to look exactly the same for everyone. In a young child, it might involve not pointing with a finger and showing less interest in playing with others. In a teenager, it’s more common to see a literal understanding of what’s said, difficulty in reading the intentions of peers, or feeling overwhelmed by the group’s unwritten rules. In an adult, the problem may involve small talk, irony, professional relationships, or exhaustion after intense social interactions.

Repetitive Behaviors and Sensory Processing

The second pillar consists of limited and repetitive behaviors. To diagnose this condition, at least the following are required: 2 of 4 types:

Motor or linguistic stereotypes – for example, waving one's arms, swaying, or repeating the same phrases.

A strong need for stability and predictability – difficulty changing plans, attachment to routines, stress caused by sudden changes.

Very limited or intense interests – passions of extraordinary intensity, sometimes very specific and all-consuming in everyday life.

Unusual sensory reactions – hypersensitivity or reduced sensitivity to sound, light, touch, smell, temperature, or pain.

Sensory processing deserves special attention, as it often has a significant impact on daily functioning. A child may cover their ears at ordinary noises or reject certain textures in clothing or food. An adult may avoid shopping malls, public transportation, or open-plan offices because the overwhelming number of stimuli causes sensory overload. To an outsider, this may sometimes seem like an „overreaction,” but in reality, it can be a genuine barrier to daily functioning.

It's also worth remembering that the answer to the question, What is the autism spectrum?, is not limited to a single characteristic. A diagnosis requires a full set of symptoms, their developmental history, and an assessment of their impact on daily life.

What symptoms may be observed in children, adolescents, and adults?

Symptoms of ASD may be noticeable very early on, but they are not always immediately obvious. For some children, the first signs appear before the age of 2. For others, the picture only becomes clear when social demands increase, such as in preschool, school, or the workplace. Therefore, the time of diagnosis and the time when symptoms first appear are not always the same.

Early Signs Before Age 2

During the early stages of development, specialists look for several warning signs. This does not mean that each of them indicates ASD, but rather that their presence may warrant a more thorough evaluation.

No or few gestures, such as pointing, waving goodbye, and showing objects to others.

Less eye contact or difficulty focusing their attention together with an adult.

A slight reaction to the name, despite having normal hearing.

Delayed Speech Development or an unusual way of using words.

Less need to play together and limited sharing of joy with others.

Repetitive movements or a very strong attachment to certain patterns.

In practice, parents often describe it this way: the child is more interested in objects than in people, arranges things in a specific order, gets upset when the routine changes, doesn’t show what they want, or doesn’t spontaneously seek social interaction. These are examples, not a definitive diagnosis. A reliable assessment always requires considering these observations in the context of the child’s overall development.

Masking Symptoms in Older Adults

In older children, teenagers, and adults, symptoms may include masked. This means that a person learns social behaviors „by heart,” observes others, and consciously adapts to their surroundings. From the outside, this may appear to be effective functioning, but it often takes a great deal of energy and leads to overload, tension, and exhaustion after social interactions.

For this reason, ASD is sometimes mistaken for shyness, social anxiety, perfectionism, eccentricity, or „just being that way.” Someone may be a good student with a rich vocabulary and yet still struggle to understand allusions, group dynamics, or emotional nuances. Someone else may be perceived as withdrawn for years, even though the main issue is actually difficulty interpreting social cues and sensory overload.

That's one of the reasons why the question What is the autism spectrum? It often does not appear until adolescence or adulthood. This is not because the symptoms „started late,” but because they were not properly diagnosed earlier or were effectively concealed.

⚠️ One symptom isn't enough: A lack of eye contact or a speech delay does not automatically mean ASD. The entire developmental and functional profile is assessed.

What Is the Diagnosis Process for Autism Spectrum Disorder Like in Poland?

Diagnosing autism spectrum disorder in Poland is a process multi-stage and interdisciplinary. It is not based on a single test, a brief consultation, or a single symptom. Its purpose is not only to confirm or rule out ASD, but also to describe, exactly how a person functions and what kind of support he needs.

Polish diagnostic standards and recommendations refer to DSM-5-TR and ICD-11. In practice, this means that the specialist considers both the core symptoms of ASD and language, cognitive development, sensory processing, independence, and comorbid conditions. This approach provides a more complete picture than a simple „yes” or „no” answer.

How the Interview and Observation Are Conducted

The first stage is usually detailed developmental interview. For a child, the discussion with the parent or caregiver covers pregnancy, childbirth, motor development, speech, social behavior, play, sensory responses, sleep, and difficulties in daily life. For adolescents and adults, both their childhood history and their current functioning at school, work, in relationships, and in terms of independence are important.

The next element is behavioral observation. The specialist assesses, among other things, how the child initiates contact, sustains a conversation, understands questions, responds to changes, uses gestures, expresses emotions, and exhibits repetitive behaviors. With younger children, part of the observation takes place through play. For older children, conversation, tasks, and the analysis of everyday situations play a greater role.

In many cases, feedback from daycare, preschool, school, or from a partner and loved ones can also be helpful. While they do not replace an evaluation, they provide insight into how a person functions in different settings. This is important because symptoms of ASD can manifest differently in a quiet office than in a noisy classroom or a stressful work environment.

Which specialists are involved in the process?

The diagnostic process usually involves several specialists. Depending on the patient's age and circumstances, these may include:

psychologist – conducts interviews, observations, and assessments of cognitive and social functions,

child and adolescent psychiatrist or adult psychiatrist – is responsible for clinical evaluation and medical diagnosis,

speech-language pathologist or neuro-speech-language pathologist – evaluates language, communication, and the pragmatics of speech,

special education teacher – describes educational and adaptive functioning,

sensory integration therapist or other sensory specialist – analyzes the response profile to stimuli.

Not every diagnosis is the same, but best practice is to avoid limiting yourself to a single perspective. If the symptoms involve speech, relationships, concentration, and sensory responses all at once, you need a multifaceted view—not just a single opinion.

Why are support needs also assessed?

A modern diagnosis does not end with determining whether the criteria for ASD are met. It is equally important to determine, What kind of support do you need right now?. For one child, communication therapy and support at preschool will be key. For another, strategies for coping with change and sensory overload will prove more important. For an adult, the main focus may be the work environment, relationships, or the nature of co-occurring anxiety.

In practice, therefore, it is not only the symptoms that are assessed, but also their impact on daily life. This approach is more useful than a diagnostic label alone because it leads to concrete decisions: what activities to implement, what accommodations to make, what to avoid, and how to build a future plan based on the child’s strengths.

✅ Prepare for the diagnosis: Write down examples of behaviors, developmental history, sensory responses, and relationship difficulties. This facilitates a thorough diagnostic interview.

How common is ASD, and why are some people diagnosed later in life?

Epidemiological data show that ASD is not a rare condition. Worldwide, The median prevalence among children is approximately 1–1.5%, or roughly 10–15 children per 1,000. In Europe, the results often fall within the range of 6–12 per 1,000, although the specific figures depend on the country, the survey methods, and the availability of diagnostic tests.

Data from around the world, Europe, and Poland

Differences in statistics do not necessarily mean that children are actually born with ASD more often in one country than in another. This is often determined by three factors: How wide is the screening process?, How well does the diagnostic system work? and What criteria and tools are used?. The better the access to evaluation, the greater the chance that people with less typical symptoms will not be overlooked.

Poland still lacks a single, up-to-date population-based study that would provide a straightforward answer for the entire country. However, there is a noticeable increase in the number of diagnoses and growing awareness of the problem. System-wide recommendations increasingly point to the need for broader developmental screening of children, as earlier detection of difficulties shortens the path to support.

Why Are Girls Sometimes Diagnosed Later?

ASD is diagnosed more often in boys. In studies, the difference is sometimes estimated at as much as 3–5 times. However, this does not automatically mean that girls are actually much less likely to fall within the spectrum. Some of them simply harder to notice in the standard diagnostic model.

The reasons are quite specific. Girls are more likely to mask their difficulties, adhere more closely to social norms, and are able to conform to them despite great effort. Their interests may be intense but socially acceptable, so they are less conspicuous. Instead of obvious stereotypical behaviors, they are more likely to exhibit overwhelm, anxiety, withdrawal, or perfectionism. As a result, diagnosis is often delayed, sometimes until adolescence or adulthood.

This is also important because a delayed diagnosis increases the risk of developing secondary difficulties, such as low mood, chronic social fatigue, anxiety disorders, or self-esteem issues. The longer someone tries to function without understanding the cause of their difficulties, the greater the psychological toll may be.

What Is Known About the Causes and Risk Factors of ASD

One of the most common questions is: What causes ASD? The answer is, frankly, complex. There is no single cause. Current scientific knowledge suggests that multifactorial nature, that is, the interaction of genetic predisposition and certain prenatal and environmental factors. This means that it is impossible to identify a single, simple „reason” that explains all cases.

The Role of Genetics

The strongest and best-supported one is genetic component. Research shows that other neurodevelopmental and mental disorders, including ADHD, are also more common in the families of people with ASD. This means that some biological risk factors may be common to several diagnoses, not just autism.

However, genetics doesn't work like a simple „on or off” switch. It usually comes down to many genetic variants, which, to varying degrees, affect brain development, sensory processing, attention, and social communication. For one person, family factors may be more significant, while for another, additional prenatal stressors may play a greater role.

Prenatal and environmental factors

Large research reviews identify several factors that may be associated with an increased risk of ASD. These include, among others, gestational diabetes, Prenatal exposure to certain medications, short intervals between pregnancies and maternal obesity. However, it must be emphasized very clearly: these are risk factors, rather than specific causes.

This is an important distinction. If a given factor increases the risk, it does not mean that it automatically leads to ASD. Similarly, the absence of these factors does not guarantee that ASD will not occur. In medicine, this model is common: risk results from the overlap of many factors, rather than from a single, simple mechanism.

In practice, the most reliable approach is as follows: What is the autism spectrum? From a causal perspective? It is the result of the complex interplay of biology and development, not the result of a single parental decision, parenting style, or isolated event.

What Conditions Often Co-occur with Autism Spectrum Disorder, and Why Early Intervention Matters

ASD very often does not occur in isolation. Many people also have other neurodevelopmental, mental health, or medical challenges. This is one of the reasons why a good diagnosis should be comprehensive. If you focus solely on autism itself, it’s easy to overlook the factors that most significantly impact daily life.

The Most Common Comorbid Disorders

The most commonly reported comorbid difficulties include:

ADHD – problems with concentration, impulsivity, and attention regulation,

intellectual disability – for some people, it affects their learning pace and independence,

anxiety disorders – especially in the face of social stress and unpredictability,

sleep disorders – difficulty falling asleep, waking up, and recovering,

low mood and depression – especially among teenagers and adults,

mood disorders, including bipolar disorder – The risk is higher than in the general population.

The research data is clear on this point: by the age of 30 ponad połowa osób ze spektrum może otrzymać diagnozę depresji lub zaburzeń lękowych.
